to become a full time practitioner and have enough clients to be working 5 days a week?
Or should i say, how long did it take you to become full time?
And how many clients can be done each day on average, for dm visits or in a clinic?

Andy Hi Andy, You can see pts every 20 minutes in a clinic, provided you have two treatment bays, and someone to man reception and deal with instrument sterilisation. This assumes a competent and practiced practitioner. Home visits obviously take longer since you have to factor travelling time in. I become full-time after three years. For full-time read four days a week. At an average fee of £25.00 (todays figures), seeing 3 pts an hour and working a 6-hour day equates to around £450 a day. Thats £1800 a week, with less risk of RSD. Remember than no-one became rich as a Podiatrist/FHP, but there are plenty of Pods/FHPs with RSD . My purely hypothetical example depends upon a full diary, with no pts cancelling or dying at short notice. This really depends upon your locality, the amount of competition around you, and how good you are. I did a couple of years in the NHS to knock of the rough spots in my treatments before going into Private Practice - unfortunately this is a luxury FHPs do not have. Hope this helps. Regards,

You clinic turns over people pretty quickly then at 20 mins per patient. I'm working on a treatment time of around 30 mins as trying to put a small business plan together, hence the reason for the questions!
My aim was for a turnover of around £100 -£120 per day, is this reasonable/acheivable with domiciliary? would love a clinic ultimately though
Am expecting an application pack to apply for the BSc course as Salford Part time starting January. need to shadow a podiatrst prior to the interview if i get one!

Hi there! Only the newest of newbe's myself, started up in April. Working around young children so only doing school hours, and the average appt takes me 1 hr!! Sooooo sloooow!!!! But I am already earning £100 a day, including travelling etc as only dom at the moment. Loving every minute though! And not so worried about fitting in so few a day (4), as they are mostly new Pts still, and I know I'll get quicker with time Karen

Hi All A very wise old accountant told me when I first started in practice, " If it does not work in 5 years then it is not going to and that is the time to walk away" I think that is basically true but it also makes the point DON'T think now ,plan for the future. You will always get peaks and troughs in patient flow some seasonal and some ,well there's no reason why but the secret is dont panic it will pass and the phone WILL ring again. Keep a monthly patient flow chart and an end of year total for new patients ( mine was 780 last year). Bear in mind also you will lose a few as well through natural wastage and hopefully by curing them !! ;D That may seem like a downer that the patient is not coming back BUT the referrals you will get from that one patient will reap it's own rewards (= 780 new patients which are mostly from recommendation) but obviously it will take you several years to reach that number, but if I have you can ;D Karen, well done on building your patient base ;D Nooooooooooooooooooooooooooooo ;D ;D That is far tooo long!! Think about that and your traveling time on top ( assuming that is not included in the Hour). I don't know about traffic and parking in your area but here it takes 30 minutes to get anywhere SO, add that on both ways then you are now taking 2 hours on each patient so your £25 per visit is now worth £12.50 per hour less tax insurance consumables equipment replacement car running costs phone bills............ Now look what you actually have left in your pocket in real terms Yes frightening isn't it My advice = cut out the cups of tea and the cosy chat as when you do become busier you wont have time for that. Always appear to be busy, concentrate on doing a proper job but efficiently ( aim for 30 minute appointments). Efficient time management will become more and more important to you as you get on so start as you mean to go on Just my thoughts hope it helps cheers Derek

The theory in my head is an average of 30mins appt and 30mins travel over the day, obviously this will vary, aiming for 6 or 7 maybe appts depending on availability, location etc
Is it common place to charge more for the first appoinment as an assessment, or do most people charge the same regardless??

Hi Andy I do charge more for the initial consultation and treatment as I have the extra computer input on the practice management system to do which again is using my time = patient pays for it. If you follow protocols the paperwork alone and you knowledge of the assessment protocols is why they must pay a bit more on the initial visit. How much more is up to you Cheers Derek

Fair enough, it sounds right to me that the inital assessment visit should be charged out as more,with follow ups at a standard price. In my experience of private healthcare this is the norm.
How much extra is normally charged for a domicilary visit over a clinic visit?

I charge £55 dom £40 surgery But thats because I dont want doms !! ;D all are happy with the surgery fee coz I can do more podiatry in the surgery which I cant do in a dom situation D

I HATE it Standing on peoples front door soaking wet and cold is not my idea of a good living On top of which steralising everything I stand up in in the name of infection control which takes hours to do is suddenly destroyed when the poxy dog jumps up slobbering all over me and then i look round mid treatment and the cat is sniffing all my instruments after licking its arse !!!! That is besides the poo I very often find on their feet or on the chair I've just sat in or Ive trodden in On top of which these energy saving bulbs are worse than sex for making you go blind So Do I like Doms NO !!! ;D D
