Time, methinks, for a fresh biomechanics thread.
First some definitions (so we are all talking about the same thing).
Forefoot post - An extrinsic modification under the distil medial end of a 2/3rds length orthotic shell
Forefoot varus extension - A wedge applied which starts under the distil end of the shell / valgus pad and extends under the 1st MPJ.
Intrinsic wedges are a whole other thread so we shall put those to one side for now.
1. Do you use them? 2. If so why and when? 3. If not why not? 4. Are there any specific times you feel they are contra-indicated. This one is half a question, half an argument
5. If you use a forefoot wedge on a balanced orthotic is it truly a forefoot modification of is it de facto either a wholefoot wedge or an intrinsic modification depending on the manufacture technique?
What say you?

Do you use them? No. Not used extrinsic forefoot posts for over 10 years. Everything is intrinsic.
Extrinsic wedge Not used one. Mainly because I have not needed to use them. Have used a Moreton's extension and reverse extension and Kinetic wedge, which admittedly is not the same thing.
