The trouble with feeding the beast is that ultimately, we will all be required to feed it even if not intimately acquainted with it. Some time in the dim and distant future, Pod Smith PP gets taken to court by A.Litigious Pt. Mr Bustard (Litigious's barrister) asks whether Smith has fulfilled the generally accepted standards of the profession. Smith replies that there is guidance only, not requirements. Bustard then produces evidence that in the NHS - and when all said and done, private medicine ALWAYS (emphasis) follows NHS procedures - all employed pods. are required to provide a printed audit trail. Will the HPC support Smith? Will Smith's professional body support him? I don't know, but I bet Smith gets dinged and I bet he whinges now that the beast has bitten him. All the best Bill i agree, but again not the fault of the NHS and bench top sterilization is sooooooo 2008, disposable or outsourced sterilization is the new black to drop one bollock could be seen as careless........ ;D

Hi Ron Another bloody fine waste of public funds IMHO. AND according to my learned friend Roberts mindset.... Where's the proof that they were sterilised properly in the first place ? and with the required transportation to the end user what is the probability of contamination?? i suppose the argument from the company would be along the lines of..... ;D ;D i'm guessing that we will get individually packed sets of instruments, guess is the operative word as we haven't been told and we are supposed to be impementing this in 2 weeks time

Hi Ron Exactly M ludd so where's the difference in me doing the steralising and the company doing it coz the use basically the same vacuum autoclaving system as I do My learned friend can fill you in on that one as his trust has started using that very system But hearsay has it M lud the choice of nipper , blacks file leaves a lot to be desired . More likened to the finesse of treating hooves rather than feet apparently ;D Cheers Rumpole

I thank my learned friend. He is indeed correct. The nippers are, in fact, not that bad. The blacks files are huge monstrosities which i would not inflict on any of my worst enemy(s) and the thwaites nippers likewise. Files, burrs, handles forceps etc are actually quite acceptable. Don't tell but i've been sterilizing my own blacks files at home, pouching them, and taking them to work for any patients i don't want to actually torture! However i have it on pretty good authority that following the noble, grand and sweeping initiative to rid the world of the bench top sterilizers which were perfectly adequate for the last 30 odd years we may be returning to them. Perhaps the finance department did the maths on approx 25 000 or so patient contacts a year * £5 per pack in a department with only 6 or 7 full time members of staff! At the moment the dept is paying more per hour for the instruments than the clinician using them . It would be funny if... actually no its not funny! Utterly utterly ludicrous. In a rational universe we would have disposable dressing and wound debridement packs (where it really matters) and perhaps doms packs, CCSD for nail surgery packs and bench top for "routine" podiatry. But hey, that would involve allowing the clinical level managers to make decisions. Never happen. Regards Robert Who has just been asked to perform a PNA on a 10 year old with P**S poor equipment and had to dig the old kit out the cupboard and sterilize it instead cos he could'nt use the disposables and still look the patient in the eye! *thud* Was that what i thought it was?
